Advising Forms

This page offers a collection of advising forms for the various English majors and minors.

• Application to become an English major.
• Checklists to be used in consultation with your advisor to determine whether you are meeting the requirement for an English major or minor.
• Suggested plans of study (four-year and transfer) that you can use to plan which courses you will take and when.

Download the Checklist, Four-Year Plan of Study or Transfer Plan of Study forms, fill them out and then bring them to your appointment with your English Department Advisor.

The Application can be handed in (WIH 315), mailed to the English department (Campus Box 4240, Normal, IL 61790-4240), or emailed to mavegte@ilstu.edu.

The deadline for admission to the Fall 2008 semester is Wednesday, March 5. Applications received after that date will be held for review in September for Spring 2009 admission.
